Details: Harvest Homes, a 24-hour staffed facility, is an important addition to the continuum of care for individuals with mental illness. It bridges the gap between living independently in one’s own apartment and group home living. Harvest Homes is a HUD 811 PRAC (project rental assistance contract) which is like a project-based voucher where the voucher remains with the property. Harvest Homes property, maintenance and waiting list is managed by Housing. Housing assistance is available to eligible tenants. Harvest Home’s was opened in 1997 and became one of the first permanent supportive housing choices for adults with serious mental illness in Grand Forks. It consists of twelve one-bedroom apartments, where one unit is ADA accessible. All bedrooms along with a commons kitchen and living area are located on the ground floor, where meals and socialization are easily shared. A commons laundry room provides two washers and two dryers, all coin-operated. A van is available for transportation needs and bus training is provided and public transportation is encouraged. Limited storage units are located in the basement. Supportive services are provided through partnership with Northeast Human Service Center, which includes Case Management, while Prairie Harvest Mental Health provides nursing and medication monitoring, activities of daily living, employment support and groups. Harvest Homes is suited for individuals who desire independent living in their own apartment but prefer or require the support of 24-hour staffing. All apartments consist of a bedroom, bathroom, living room, and kitchen. Tenants need a moderate amount of skills ranging from cleaning and cooking to washing dishes and laundry. Staff support includes prompting, instruction, training and hands on direction. Applicants with limited mobility can navigate this property. The one ADA accessible apartment is suited to those individuals with some physical limitations, i.e. cane, walker, wheel chair

Harvest Lodge consists of 6 individual bedrooms on two floors and a shared kitchen, living room and laundry areas. Stairs are the only way to navigate between the two floors. This residence was made available to Prairie Harvest Mental Health through a HUD grant. Since Harvest Lodge is a project-based housing option, the voucher remains with the property and Harvest Lodge property, maintenance and waiting list is managed by Housing. Supportive services are provided through partnership with Northeast Human Service Center, which includes Case Management. Prairie Harvest Mental Health provides support with nursing, activities of daily living, employment and groups. Residents are employed at a business run by Prairie Harvest Mental Health. Harvest Lodge follows the Fairweather Lodge Model, a national model developed by Dr. George Fairweather for adults with serious mental illness. The philosophy driving this model is that people who live together and work together can more easily recover and find stability with support from their peers. Harvest Lodge is suited for those individuals who are capable of working and living together with minimal staff support. Support services are available on a limited basis. The Lodge resembles a family with each member contributing to the maintenance and care of the house and each other. Members share in managing the home, including domestic chores and the purchase and preparation of food. Members may also, prior to placement on the waiting list, approve who is accepted to live at the lodge by way of majority rules decision-making. Lodge fees assessed toward all tenants pay for groceries, cable, newspaper and other amenities.

Details: St. Anne’s Housing for the Elderly offers 30 efficiency apartments on two floors in our north wing. These are open to individuals with limited income who are 62 or older or disabled. Rent is based on adjusted monthly income and the rest of the expense is subsidized by the federal government’s Housing and Urban Development (HUD) program. Persons of all creeds, ethnic backgrounds and races are welcome. A financial review and income verification is required for eligibility and rent assistance. The units have temperature controls, kitchenettes, private baths, emergency call switches, and spacious closet and storage space. The units are unfurnished so that residents may bring familiar belongings. There are laundry facilities on the second floor and a beautiful spacious lounge on the first floor. The building has fire resistant construction throughout, with smoke alarms in each unit and an automatic sprinkler system. Parking space is available to those with cars. There is staff on duty 24 hours a day (in the Basic Care wing) to ensure the safety of each resident. Apartment residents are invited to take part in activities offered at St. Anne’s at no additional charge. They may also purchase such extra services as nursing assistance and meals in the main dining room. Laundry and housekeeping services may also be purchased.
